Privacy Policy

This website is operated for Patuxent River Dental Care, 13916 Baltimore Ave, Laurel, MD 20707. It exists so that people considering dental implants can check whether they may be a candidate and request an appointment. This policy explains what we collect on this site, why, and what we do with it, in plain language.
What we collect
When you complete the candidacy check or request an appointment, we collect the information you type in and the answers you choose:
- Your first and last name, phone number and email address
- Your answers about your teeth, how long you have had the problem, what you have tried, and your readiness
- If you choose to answer them, your interest in payment plans and a general credit range. These questions are optional and one option is always "I would rather not say"
- The appointment day and time you request
- Whether you agreed to receive text messages
We also record technical information about your visit: the page you were on, the time, and if you arrived from an advertisement, the campaign identifiers in the link. We use this to understand which advertising brings people to the site. This information does not identify you on its own.
Why we collect it
- To reach you about the appointment you requested and to confirm the time
- To let Dr. Syeda's team see your answers before you arrive, so the visit is about you and not about paperwork
- To show you the payment options that fit, if you asked about them
- To measure and improve the website and our advertising
We do not sell your information. We do not share it with other dental practices, marketers, or data brokers.
Text messages
If you check the box agreeing to texts, we may text you about your results, your appointment, reminders, and follow-up from the practice. Message frequency varies. Message and data rates may apply. You can stop at any time by replying STOP to any message, and you can get help by replying HELP or calling (301) 498-6511. Agreeing to texts is not a condition of receiving care or an appointment.
Who handles your information
Your information is stored and processed by service providers who work on our behalf and are permitted to use it only to provide their service to us: a cloud database that stores your submission, an automation service that routes it to the practice's records and sends messages, and the messaging and email services those messages are sent through. If we use advertising measurement tools, they may receive that a form was completed on a page, but not the content of your answers.
Health information
Some of what you tell us here is about your dental health. Once you become a patient of Patuxent River Dental Care, your health information is protected under HIPAA and the practice's Notice of Privacy Practices, which you will receive at your first visit. Before that point, we treat what you share on this site with the same care: we limit who can see it, we do not sell it, and we use it only for the purposes above.
Cookies and tracking
This site may use a small amount of local storage on your device to keep the form working if your connection drops and to keep the appointment calendar accurate. If advertising measurement is turned on, a pixel from the advertising platform may set cookies according to that platform's own policy. You can block cookies in your browser at any time and the site will still work.
Your choices
- You can ask us to correct or delete the information you submitted by calling (301) 498-6511 or writing to the address above.
- You can stop texts at any time by replying STOP.
- You can unsubscribe from emails using the link in any email.
Children
This site is for adults considering their own care. We do not knowingly collect information from anyone under 18.
Changes
If this policy changes, the effective date at the top will change with it. Material changes will be noted on this page.
